IPN appears to happen outside of the PayPal transaction in the TTG cart and requires an additional page.
From what I just read about it, it looks like it's something you need to implement by creating a special page on your site that contains the proper code.
(You can probably just copy the code — probably should use php — into a plain text editor, save the file and upload it to your server. Then enable IPN in in your PayPal account.)
