There are configurable checkbox options in the Client Response module that might serve the purpose.

The model release should include an ID (e.g. drivers license), especially to show the date of birth. It also needs a signature, which isn't easy to do on a web page. I don't know if clicking (e.g. click through licensing agreement) will hold up in court.

Another thing, you want the model release signed before the shoot (or immediately after).

I highly recommend this app. It has a very good legal contract, and you can customize it. It makes capturing the ID easy, and an easy way for the model to sign it.

There's a version of the app for you phone or tablet, so it's very convenient to get this completed during the shoot.
