Currently, you can either choose 'Random' or one of the images from the album collection.
For a private gallery, I may not want to choose one of the album's (galleries) images.
If I add my special image to the album, I can select it of course, bur it will appear as one of the photos in the album too.

You can put this image into the custom-thumbnails directory so it won't appear in the gallery. The documentation dates back to CE4, but applies to backlight too: http://ce4.theturninggate.net/docs/doku … album_sets.
I have a custom-thumbnails gallery in /galleries that is managed from Lr Publisher (disable search and indexing). This gallery uses a custom template that has the images sized according to my needs. If I need a new thumbnail, I just put the image in there, publish it, and then select it from the target gallery. This is really smooth and easy to use.
