Cart documentation is here, in case you haven't seen it. Some of your questions may be answered there.
The Cart interacts with TTG CE2 Highslide, Photoswipe, and Horizon Lightroom gallery plug ins. One of these is required.
You need to create and export the cart and the galleries from Lightroom. Use FTP for upload. Any changes you make to the galleries need to go through Lightroom.
The TTG Publisher makes creating, adding. or removing images from a gallery very easy, and Publisher managed galleries are compatible with the Cart.
But still, Lightroom is required to manage the galleries.
Editing in Photoshop shouldn't be a hindrance, as long as you're using Lightroom to manage it all.
TTG galleries can be password protected so you can easily have public and private galleries, all connected to the cart system.

There are no recurring fees, if that's what you mean.
