How come it works for sportfoton.se but not for ishockeybilder.se.
sportfoton.se has the following path on the site www.sportfoton.se/phplugins-sample
and in the Lightroom Gallery Templates is the path
/customers/9/9/f/sportfoton.se/httpd.www/phplugins.

if this is the server path to phplugins on the sportfoton site:
/customers/9/9/f/sportfoton.se/httpd.www/phplugins-sample
then that's what needs to be entered in the gallery template's path to phplugins field, not:
/customers/9/9/f/sportfoton.se/httpd.www/phpluginsif this is the server path to phplugins on the ishockeybilder site:
/Customers/9/4/f/ ishockeybilder.se/httpd.www/phplugins-sample
then that's what needs to be entered in the gallery template, not
/customers/9/9/f/ishockeybilder.se/httpd.www/phpluginsEither that or change the name of the folders from phplugins-sample to just phplugins, like the documentation directed.

All I can tell from what you've told me is that you're not actually using phplugins because you're not using the correct path.
Have you tried correcting the path? The easiest way would be to simply change the name of the phplugins-sample folder to phplugins.
Even then You could still have problems, but it would be an obvious first step in troubleshooting and would eliminate one possibility.
Could be that it's a server issue
