I installed CE4 Publisher in LR 5 last night and all is working seemingly fine, but I am confused as to exactly what the "mark to republish" feature does. I have played with it but it seems to have no affect.

when you make an edit adjustment or metadata change to an image or change its order in the gallery, you then need to mark it for republishing. Then just click on the Publish button and the image will be re-published with its updates

Ok. so it applies to individual photos, and only the changes will be published? Is there indication that a photo has been marked?

Yes. In Lighroom's Publish service any images that have been marked for republish or deletion are segregated from the rest. Just give it a try, you'll see what I mean.
A hint to speed things up if you've only made metadata changes is to check the box in Publisher set up labeled "Push Metadata without updating existing photos". This way, only the metadata for the image is updated without the need of uploading the photo itself.

When updating images or image metadata, the images will usually be automatically added to the "Modified Photos to Re-Publish" queue. You can use the "Mark to Republish" option to force images into the queue, which is sometimes useful to do.

One of my wishes is that Lightroom would provide the developers with more information about why an image is automatically marked for republishing. As it currently works, we can't tell whether an image is marked for republishing due to changes in the metadata, or due to changes that affect the physical appearance. The former only requires metadata to be pushed, while the latter requires a new image rendition to be pushed. It would be nice to be able to automate the type of publishing needed.
