In LR's Publish Services, assuming you've already setup CE4 Publisher, create album sets within an album set, depending on how you want to organize things.
For example, you might do this:
Client Name (set)
-- Client Selection (set)
-- gallery
-- gallery
-- Delivery (set)
-- gallery
-- gallery

Or this:
Client Selection (set)
-- Client Name
-- Client Name
Delivery (set)
-- Client Name
-- Client Name

You might want to disable breadcrumbs on these sets and albums, so that clients cannot wander back to a level higher than you want them to. Any of the above setups would allow you to drag a gallery from the Selection set into the Delivery set when ready.

Or you might setup multiple publisher instances, and publish to entirely separate locations. In such case, you might be able to go ahead and keep breadcrumbs enabled. But I don't think you can drag albums between publish services, so you'll need to think about how you want to organize that. For example, for each client:

To wit, you can do whatever you want. Our system is a series of components, which allows you to deploy things in a variety of ways, depending on how you want to organize your site. We try to dictate as little as possible, because what we've learned in our business is that every photographer is different, and every photographer has different needs, and every photographer wants to do things in their own, different way ...
Whatever you do, setup your navigation accordingly. You can use PHPlugins to manage navigation at the code level, which I think makes it a lot easier to change up navigation on-the-fly, without having to export stuff from Lightroom again. We have documentation on how to do all this, and most of the code has already been written for you, so it's a mostly a copy-and-paste-and-edit job.

For password protection, you have options. To start, enable password protection features for the template in the Web module; it's under the Output Settings pane. When creating your album in publish services, you will be able to enable password protection. Or you can set the album as "Client Managed", in which case you need to create a client account in TTG BE, then assign the client albums.
